Keto breath is not coming from your teeth. It is acetone, made in your liver, carried in your blood and breathed out through your lungs, which is why brushing, scraping and mouthwash barely touch it. That single fact explains most of the confusion around this topic, and it is the reason the mints and sprays sold as a fix do not work for long.

Acetone makes up only about 2 percent of the ketones in your blood, but it is small and volatile, so it crosses from blood into the air sacs of the lungs and leaves unchanged. The smell people describe as sweet, fruity, or like nail polish remover is a systemic signal, not a hygiene problem.

Where the smell is made

When carbohydrate is restricted, the liver shifts to producing ketone bodies as fuel. Three are made. Beta hydroxybutyrate accounts for roughly 78 percent of what circulates, acetoacetate about 20 percent, and acetone about 2 percent.

Acetone is the odd one out. It is a small molecule with a low boiling point, so it moves easily out of blood and into the alveoli, the tiny air sacs where gas exchange happens. You then exhale it. Musa Veloso and colleagues, publishing in the American Journal of Clinical Nutrition in 2002, showed that breath acetone is a reliable indicator of ketosis in adults eating ketogenic meals.

This is why the usual advice fails. Brushing, flossing, scraping the tongue and rinsing all work on the mouth. Acetone is being delivered continuously from the bloodstream through the lungs. Clean the mouth all you like and the next breath brings more acetone up from below.

The numbers on breath acetone

Reported concentrations give a sense of scale. Normal breath acetone is around 3 micrograms per litre of exhaled air. On a ketogenic diet drawing more than 90 percent of energy from fat, it plateaus at roughly 150 micrograms per litre. In prolonged fasting it climbs to around 300, about a hundredfold increase on baseline.

Fasting studies also describe the timing. Breath acetone rises gradually from the first day, climbs sharply at around 50 hours, and levels off by about day four. That is a useful pattern, though it comes from fasting rather than from people eating a low carbohydrate diet at home.

One thing worth avoiding is the idea that there is a threshold reading that proves you are in ketosis. Published numbers vary with the measuring device, the collection method and the individual, and no agreed cut off exists that a consumer breath meter can hold you to.

The other bad breath, the one that is actually oral

Plenty of people on a low carbohydrate diet have two problems at once, which is why the advice online contradicts itself.

Classic halitosis comes from bacteria living in the coating on the back of the tongue and in the gum pockets. They break down proteins and release volatile sulphur compounds, including hydrogen sulphide and methyl mercaptan. That smell is produced in the mouth, so it does respond to tongue cleaning, flossing, treating gum inflammation and professional cleaning.

Low carbohydrate eating usually means more protein, and more protein means more substrate for those bacteria. So a keto diet can add a real oral halitosis component on top of the acetone. If you clean your tongue and the smell improves a bit but does not disappear, you are probably dealing with both.

How long keto breath lasts

The internet answer is two to four weeks. I would not repeat that as though it were established, because it is not.

Breath acetone kinetics have been studied over hours and days, in controlled fasting and feeding conditions. Nobody has followed a group of people eating a low carbohydrate diet in ordinary life and tracked their subjective breath odour over weeks. So the honest position is that the natural history of keto breath has not been measured. What people report, consistently, is that it is strongest in the first days and eases as the diet settles, which fits the acetone plateau pattern but is not the same as evidence.

What does change the smell reliably is changing the input. Loosening carbohydrate restriction lowers ketone production and lowers exhaled acetone. That is a trade off only you can weigh up.

Dry mouth on a low carbohydrate diet

Almost everyone starting keto notices a dry mouth in the first week or two. The mechanism is straightforward. Glycogen is stored with roughly 3 grams of water for every gram of glycogen, so depleting glycogen releases that water. Falling insulin also reduces how much sodium the kidneys hold onto, which increases water and electrolyte loss. That fluid shift is also the usual explanation for the headache, fatigue and general flatness people call keto flu, which is typically described as settling within days to about a week.

Dry mouth matters for teeth. Saliva buffers acid, clears food, and carries the calcium and phosphate that repair early enamel damage. Less saliva means more risk of decay and erosion, and it also makes breath worse because bacteria are not being washed away. On top of that, low carbohydrate eating often means fewer eating occasions, and chewing is one of the strongest stimulants of saliva flow.

Being straight about the evidence here: salivary flow rate on a ketogenic diet is essentially unmeasured in the published literature. Is keto good or bad for your teeth

Both claims get overstated. Here is what can be said with reasonable confidence.

The sugar side is genuinely good

Moynihan and Kelly published a systematic review in the Journal of Dental Research in 2014, commissioned by the World Health Organization, which screened 5,990 studies and included 55. They found moderate quality evidence that tooth decay is lower when free sugars stay under 10 percent of energy intake. The stricter under 5 percent threshold was also supported, but on very low quality evidence. The WHO guideline published in 2015 reflects that split, recommending under 10 percent with a conditional suggestion of under 5 percent.

A ketogenic diet cuts free sugars to almost nothing. Less fermentable carbohydrate means less acid produced by Streptococcus mutans and less demineralisation of enamel. That is a sound mechanism supported by population evidence. It is not a keto trial result, and no randomised trial has compared a ketogenic diet with a standard diet using new cavities as the outcome.

The parts that cut the other way

Dry mouth removes some of that protection. Low fibre versions of keto also reduce chewing and cut out plant foods, and higher plant food and fibre intake is associated with better gum health, though there is no keto specific gum disease trial to point at. Some people also lean heavily on acidic drinks, sparkling water with lemon, vinegar drinks and sugar free energy drinks, which brings acid erosion into the picture. The fair summary is that keto neither prevents nor causes cavities. It changes several risk factors in both directions, and the outcome depends on what you replace the carbohydrate with and how dry your mouth gets.

When the smell is a warning

Nutritional ketosis and diabetic ketoacidosis are different things, but they smell similar, and that matters.

Diabetic ketoacidosis is a medical emergency. Warning signs include a strong fruity or acetone smell on the breath together with excessive thirst, frequent urination, nausea, vomiting, abdominal pain, rapid breathing, or confusion. Anyone with diabetes, particularly type 1, who notices that combination needs urgent medical assessment rather than a dental appointment. Breath ketone measurement has been studied specifically as a diagnostic marker for diabetic ketosis, which tells you how seriously the signal is taken clinically.

Why does my breath smell weird on keto?

Your liver is producing ketones, and one of them, acetone, is volatile enough to pass from blood into the lungs and be exhaled. The smell is sweet or solvent like rather than rotten. It is produced in the body, not on the teeth, so it is not a sign that your oral hygiene has slipped.

How long does keto breath last?

Nobody has properly measured this. Breath acetone studies run over hours and days, not weeks, and no research has tracked subjective keto breath over a longer diet. Most people report it is worst early and eases as the diet settles. Eating slightly more carbohydrate lowers it predictably.

Does the keto diet cause bad breath permanently?

Acetone breath stops when ketosis stops, so it is not permanent. If a smell persists after you return to normal eating, it is more likely bacterial and worth having checked, since gum disease and tongue coating are the usual causes.

Is keto good or bad for your teeth?

It cuts free sugars, which is the strongest dietary factor for decay, and that part is helpful. It can also reduce saliva and, in low fibre versions, cut plant foods and chewing. There is no trial measuring cavities on keto, so treat both the enthusiastic and the alarming claims with caution.

Why is my mouth so dry on a low carb diet?

Depleting glycogen releases the water stored with it, and lower insulin means the kidneys hold onto less sodium, so you lose more fluid and electrolytes early on. Drink more water than feels necessary in the first fortnight, and speak with your doctor before changing electrolyte intake if you have kidney or heart conditions.

What to do about it

Drink water steadily through the day. Keep brushing twice with a fluoride toothpaste and cleaning between the teeth, because that deals with the bacterial half of the problem even though it will not touch the acetone. Sugar free gum stimulates saliva. Skip the acidic drinks you have been using as a treat, or at least keep them to mealtimes. If the smell is bothering you enough to change your diet, ease the carbohydrate restriction rather than buying stronger mouthwash.
