If your household drinks tank water, bottled water or reverse osmosis filtered water instead of Sydney tap water, you are not getting the background fluoride that tap water supplies, which simply means fluoride toothpaste is doing all of the protective work and how you use it matters more.

None of these water choices damage teeth, and nobody needs to change what they drink because of this article. What is useful is knowing which of the two things that matter for teeth, fluoride and acidity, each source affects, and adjusting the rest of your routine accordingly.

What is actually in Sydney tap water

In NSW, fluoride is added to public water supplies at a level of 1 milligram per litre, in line with National Health and Medical Research Council recommendations. One milligram per litre is the same as 1 part per million. The NHMRC recommends a range of 0.6 to 1.1 milligrams per litre, and the Australian Drinking Water Guidelines set a maximum of 1.5.

It is actively controlled rather than incidental. NSW Health monitoring data has shown 94 per cent of more than 31,000 daily samples falling within the required range, with a median of 1.0 milligram per litre. The practice sits under the Fluoridation of Public Water Supplies Act 1957, and around 89 per cent of Australians have access to fluoridated drinking water.

Tank water and fluoride: the Australian numbers

An Australian Dental Journal study measured the fluoride content of 123 rainwater tank samples across Australia. All of them contained negligible fluoride, with a mean concentration below 0.01 parts per million and a range up to 0.18. There was no meaningful association with tank material, tank age, roof material or gutter material. The authors concluded that people relying solely on tank water for drinking may need additional exposure to fluoride for optimal decay prevention.

Compare that with the roughly 1 part per million in the reticulated supply and the picture is clear: rainwater is essentially fluoride free. That is a statement about what is absent, not a claim that tank water harms teeth. It does not.

It is also worth being honest about who this applies to. Melrose Park and most of Sydney are on the mains supply, and sole reliance on tank water is far more common on the city fringes and in regional NSW. If you have a tank for the garden and drink from the tap, none of this changes anything.

Filters: which ones remove fluoride and which do not

Carbon and jug filters

Most jug filters, most fridge filters and most carbon block systems remove very little fluoride, commonly cited at under 10 per cent. The reason is mechanical: activated carbon works largely by adsorbing organic compounds and chlorine, and the fluoride ion is small and does not behave that way. Bone char and activated alumina media are the exceptions among the common filter materials, and they do reduce fluoride.

Reverse osmosis and distillation

Reverse osmosis pushes water through a semi-permeable membrane and removes the large majority of dissolved minerals, fluoride included. Figures commonly cited sit around 85 to 95 per cent or higher for a well maintained membrane, with some manufacturers claiming more. Distillation also removes fluoride.

Two cautions. Most of those removal percentages come from manufacturer material rather than independent testing, so treat them as approximate and check the certified performance claim for your specific system. And multi-stage systems vary enormously: some include a reverse osmosis stage and many do not.

None of this is an argument against filtering your water. People filter for taste, sediment or chlorine. It just means that if the system has a reverse osmosis stage, the fluoride goes with everything else.

Bottled water, labels and the FSANZ rule

Under the Australia New Zealand Food Standards Code, fluoride may be added voluntarily to non-carbonated packaged water, to a total of between 0.6 and 1.0 milligrams per litre counting both naturally occurring and added fluoride. Where fluoride has been added, it must be declared on the label.

Two useful consequences. Because it is voluntary, most bottled water sold in Australia is not fluoridated. And because labelling is mandatory where fluoride has been added, you can settle the question by turning the bottle around. If the label does not mention it, assume there is none.

For a household that has switched from tap to bottled water as the everyday drink, that is one protective factor removed. Not a disaster, and not a reason to switch back, but a reason to be more deliberate about toothpaste.

The second variable: acidity

Fluoride is only half the story, and the half that gets ignored is acid. An Australian study of 42 commercially available bottled waters found 81.0 per cent were considered erosive to dentine and 73.8 per cent erosive to enamel. Flavoured waters were the most acidic category, with a mean pH of around 3.30, followed by sparkling waters at around 3.63, then spring, artesian, mineral and alkaline waters. Every sparkling and every flavoured water tested showed erosive risk to both enamel and dentine.

Enamel begins to demineralise at around pH 5.5. So a drink at pH 3.3 is well below the threshold, and the question becomes how long the mouth spends there. A bottle drunk with lunch is a short exposure. The same bottle sipped across three hours at a desk is a long one, and that pattern matters more than the volume.

Why receded gums change the maths

The critical pH for exposed dentine is higher, around 6.7, because dentine is less mineralised than enamel. If gum recession has exposed root surfaces, those surfaces start dissolving at acidity levels that enamel shrugs off. That is why the same habit can be harmless in one mouth and cause visible wear in another. Our article on enamel erosion, its causes and prevention covers how that damage builds up.

To be fair to sparkling water, it is not soft drink. It lacks the sugar, and the overstatement does not survive contact with the evidence. It is acidic enough to be worth managing rather than worth fearing. Why toothpaste is doing most of the work

Here is the part that makes the rest of this article less alarming than it might sound. Water fluoridation was introduced when fluoride toothpaste was not in general use. Today it is close to universal, and reviews of the evidence have found the additional benefit of fluoridation is smaller in more recent studies than in older ones. Being honest about that is more useful than quoting a decades-old percentage as though it applies to a Sydney child today.

What follows is that the topical fluoride you apply twice a day is the dominant source, and it is entirely within your control regardless of what comes out of your tap. Water fluoridation is a background layer, and a genuinely useful one because it reaches everybody, including people who cannot easily get to a dentist. But it is a layer, not the foundation.

Practically, that means brushing twice a day with a fluoride toothpaste and spitting rather than rinsing afterwards, so the fluoride stays on the teeth for longer. Our guide to how much fluoride toothpaste children need covers the amounts by age.

What this means for children

Australian guidance on toothpaste is straightforward. Until around 18 months, brush with water only unless a dental professional advises otherwise. From around 18 months to 6 years, use a children's toothpaste at about 500 parts per million fluoride, a smear or small pea-sized amount, and encourage spitting rather than swallowing. From age 6, standard strength toothpaste at 1000 to 1450 parts per million is appropriate.

If your children drink only tank, reverse osmosis or bottled water, mention it at their check-up so it can be factored into their risk assessment alongside diet, brushing habits and family history. It might change nothing, or it might mean fluoride varnish or sealants are discussed. That is an individual clinical decision, and there is no blanket rule for every tank water household.

What this article will not do is tell you to give a child fluoride drops or tablets. Supplements are not routinely recommended in Australia, and recommending them to an unknown reader would be prescribing blind. Small habits that matter more without fluoridated water

None of these are new. They just carry a bit more weight when the background layer is missing.

  • Brush twice a day with a fluoride toothpaste, and spit without rinsing.
  • Keep acidic drinks to mealtimes rather than sipping them across hours.
  • Follow anything acidic with plain water, and wait a while before brushing rather than brushing straight away.
  • Clean between the teeth daily, since that is where decay most often starts and where toothpaste does not reach.
  • Mention your water source at your check-up so it can be part of the risk picture rather than a surprise.
