Teeth whitening with a cold sore or mouth ulcer: when to wait

If you have an active cold sore or a mouth ulcer, whitening should wait until the lip has healed or the ulcer has closed, usually about 2 weeks. Here is why, and how to tell when it is safe to go ahead.

If you have an active cold sore or a mouth ulcer, teeth whitening should wait until the lip has fully healed or the ulcer has closed, usually a delay of 2 weeks at most. Teeth whitening with a cold sore or mouth ulcer is a question I hear most often the week before a wedding, when the appointment is booked and a sore appears. A cold sore is a contagious viral infection on the lip; a mouth ulcer is a break in the lining inside the mouth that does not spread to anyone. Healthdirect notes that cold sores usually clear within 2 weeks, and Better Health Channel gives 10 to 14 days for most mouth ulcers, so the wait is short.

The short version

If the sore is active, wait: whiten once the lip has healed and the ulcer has closed

That is the whole rule, for in-chair whitening and take-home trays alike. Whitening is elective, the sore is temporary, and pushing ahead tends to produce a sore that lasts longer and a patchy result where gel was kept away from the painful area. In my experience patients who move the appointment rarely regret it.

Why a cold sore stops in-chair whitening

Cold sores are caused by the herpes simplex virus, usually type 1. Healthdirect reports that about 8 in 10 people carry HSV-1 by adulthood, that the virus stays in the body for life, and that cold sores are highly contagious, even when blisters are not present. The practical problem is the equipment: a cheek retractor holds the lips back for most of an hour, the lips are dried and coated, and the gum barrier is painted on next to the lip line. Stretching an active blister can split it, spread fluid across the lip and chin, and expose the dental team to the virus. Healthdirect lists skin damage from strong sunlight, cold or wind among the triggers for an outbreak, and the dry, stretched lip of a long dental appointment is widely regarded as another.

The same logic applies at the tingling stage. Healthdirect describes cold sores as starting with an itchy, tingling feeling before the blisters form. If you feel that tingle on the morning of your appointment, ring the practice; a rebooked session is far simpler than a blister that bursts under a retractor.

Mouth ulcers and peroxide gel

A mouth ulcer is the loss of part of the delicate lining inside the mouth. Better Health Channel notes that the most common cause is injury, such as biting the cheek, and that aphthous ulcers, the recurring kind with no known cause, affect around 20 per cent of the population. Most resolve within 10 to 14 days without treatment.

Whitening gels use hydrogen peroxide or carbamide peroxide and are designed to sit on enamel, not soft tissue. Healthdirect warns that a product with a lot of bleaching agent can irritate or burn the lips, gums and tongue, and the Australian Dental Association notes that when strong gel touches soft tissue the chemical burn often turns into an ulcer before it heals. An ulcer that is already open has no protective surface at all, so gel that would only tingle on healthy lining stings sharply, and the raw area is slower to close.

Take-home trays while an ulcer heals

Pause the trays until the ulcer has closed, then resume the course where you left off; the shade you have already gained does not disappear in a week. If the ulcer sits exactly where the tray edge rubs, bring the tray in first. A long or rough edge can be trimmed in minutes, and a tray-rub ulcer will keep returning until that is done. How to tell the sore has healed enough

A cold sore passes through tingling, blistering, weeping, crusting and finally new skin. Wait until the crust has come away and the skin underneath is intact, not merely until it has stopped hurting; a scab that is still attached will lift under a retractor. For an ulcer, the yellow-grey centre fills in and the red halo fades, and if you can eat something sour without a sting the lining has closed.

When a take-home course restarts, I usually suggest one lighter session the first night back, because lining that has just healed can be a little more sensitive to gel. Keeping both away during a whitening course

  • Use a lip balm with SPF every day. Healthdirect lists skin damage from strong sunlight among the common cold sore triggers. Our guide to sun protection for your lips explains what to look for in a balm.
  • Keep an antiviral cream at home. Healthdirect notes that creams containing aciclovir, famciclovir or valaciclovir are available from pharmacies without a prescription and are used in the first few days, from the first tingle. Follow the pack directions.
  • Never share whitening trays, gel syringes or lip products. Healthdirect's advice not to share cups, cutlery or cold sore creams applies to trays too.
  • Tell us if ulcers keep returning. Better Health Channel notes that in a small number of people they are linked to low vitamin B, folate or iron.

How long after a cold sore can I have in-chair whitening?

Once the crust has fallen away and new skin covers the area, which for most people is within about 2 weeks of the first tingle. No extra weeks are needed beyond that, but give the lip a few more days so it tolerates the retractor.

My wedding is next week and I have a cold sore. What are my options?

Start an antiviral cream from the pharmacy at once if you are still in the first few days, keep the lip protected, and let us know.
